Are we talking about AFTER character creation? Because there have been games, such as Traveler, that have the potential for your character to die before he even gets out of the creation phase, and play starts, and I've kicked the bucket that way a few times.
But second to that, probably the fastest was my 1920's Cthulhu investigator/lawyer who popped out of character creation, joined the existing party, went into a suspected cultist's house, opened the fridge and took 6D6 damage point black to the face from the shotgun trap, with the GM rolling criticals. There was basically nothing left of the character from the armpits up.
Good stories, I love reading about these types of things.
I am with JPatt on this one, I have had a character die in character creation in the Hackmaster system.
Beyond that I had a character slit its own throat and jump in a fire after being ambushed by drow. Trust me you don't want drow to take you prisoner, and the GM had it out for us. We were all level one and were ambushed with no real chance.
This is a little off topic but I remember once I had a mid level fighter riding in a balloon sort of vehicle when the party was attacked by a blue dragon. The dragon destroyed are craft right as we finished it off and we all plummeted to our death. One of the PC's was a wizard and had memorized a few feather fall spells which he cast on falling PC's.
My character fell to his death, shortly after I realized I had a ring of flight, too late. His int and wis was very low so I quess I role played that well.
