This test is very poor, I think. It doesnt distinguish between actions, for which you can be morally judged, and inaction, for which you cannot.
For instance, the zip question. There could be many reasons for why I choose not to tell someone. Perhaps I dislike them for a legitimate reason, perhaps I'm afraid of his reaction, perhaps I know he would be devestated by humiliation if I told him, whatever the fact is you cannot judge by my lack of telling hiim whether I am a truthful person. My thoughts, and therefore my reasons, are hidden from you and so cannot be used to judge me morally.
Oh please, not another person trying to over analyze a whimsical quiz. Did you even read through this post before you replied? I think you need to read what it says at the top of the quiz just before the questions, and next time... at least tell us the answer you got rather than lecture us about the ideals of questioning. If you did not take the quiz then there is no reason to reply.
