REPORT: SPACECRAFT CRASHED INTO SATELLITE
A robotic NASA spacecraft designed to rendezvous with an orbiting satellite instead crashed into its target, according to a summary of the investigation released Monday.

Sheer brilliance. This ranks right up there with the blurry hubble space telescope. You would think they would work much harder on getting this right since it is millions dollars of tax player money and it takes years to get another one up in space. But oh well, I guess its just a satellite that was wasted.
Considering NASA's government funding, it's a testament to their skill that they can put anything in space at all. A sizable portion of NASA's funding still comes from the patents they hold.
As to why mistakes and accidents keep happening, lack of testing. Testing cost money, and the US government doesn't want to spend the millions it will take to work all the bugs out of a project.
