Islam Baptism
From my understanding of Islam, baptism is not required to become a Muslam. If I am correct (if I am not correct then please correct me) all you have to say is:

I've also been told you can say this:

You must say this in front of one of the leaders of the local Mosques.

Baptism In Islam Islam Studies
If by baptism you mean a ritual washing then yes there is a sort of baptism in Islam.
There are two types of ritual washing Ghusl (bath) and wudu (ablution). Both have specific formulas and methods and are very symbolic in nature. they are performed almost daily in the case of Ghusl and becomes mandatory in certain situations eg after sexual intercourse.
Wudu is required to perform the ritual prayer (salat). and also for other things like touching words of the Quran.

Islam Baptism
The ritual purifications in Islam are essential and if neglected in certain instances may constitute a sin. However they aren't necessary for salvation.
Salvation in a nutshell can be summed up in the Koranic verses.

So the conditions are:
1. Belief or Faith which may encompass a few things.
2. Good deeds or righteous acts.
3. Enjoining Truth
4. Enjoin Patience (constancy)
These principles are all joined with the conjunction "wa" (and) meaning that they are all required together. Belief without physical expression through behaviour is useless. Individual faith and acts are useless without a reciprocal effect on a larger social community hence the enjoining of truth and patience.
