I was looking through some LDS Related sites and found this great piece of art work depicting the Plan of Salvation and thought I would share it with you: Plan of Salvation (front and back).

I've seen this before but unfortunately it's is missing a couple of things; they are the 3rd and 4th resurrections.

The third and fourth resurrection is not considered a basic teaching when sharing the Plan of Salvation. No one card this small could possibly cover everything, it is meant as a help - a guide, in my opinion, it is a great visual aide.

Honestly I don't think that the 3rd and 4th resurrections even matter when talking about the Plan of Salvation to investigators.

I just added an old Plan of Salvation leaflet to the Downloads plus section or click here as it was too big to place as an attachment here. It gives the unusual aspect of the Plan of Salvation unlike the one in the first Message.

Cool, you know Ensign bookstore has a nice chart with all the scriptural references on it. It is a bi-fold card, and it is called the plan of salvation at a glance. Granted it is not as artistic as the one you've posted, but it works!

Dude, yes, I have actually used that one on my mission, but it has long been torn up from usage. If I remember correctly the emphasis was on the scriptures and explanation more than the graphics, but it did the job.
