Speeding Up Death

'CHAIN OF ERRORS' IN ELDERLY MAN'S HOSPITAL DEATH

A "chain of errors" involving wrong medicine and mixed-up documents sped the death of an 82-year-old-man during his four-day stay in Auckland Hospital.
