Is it possible for a mother to be very thin but have a fat baby at birth?

Yes, it's certainly possible, most that I've heard of, tend to end up giving birth by caesarian section, as slim, small women tend to have small pelvis's, and this makes it very difficult for smaller women to deliver naturally (when delivering a big baby)
