When I was 10 or 11 years old, my sister and I and a friend would play this board game called Nightmare. That game would be perfect for Halloween for kids and parents. My mom and my friend's mom used to watch us play too. It was funny, because the board game came with a video tape where the person in the video was this scary character who lead players through the game, but he belittles the players, calling them names like "maggot" and stuff like that. The game was actually scary to me at that age. Below is a wikipedia link to some information about the Nightmare game and others from the company:

As for video games, all the ones mentioned above are really scary. Some other honourable mentions are Clive Barker's Undying and House of the Dead games (arcade too). Dead Space is either out now or coming out with a few days. It's about zombies in space and it's an action game. Supposedly people who have tried it were jumping in their seats, so I think I'll pick it up too.
Hellgate London was a game built on constant contact. Those hellgates, if not shut down or closed, continually spewed forth gruesome and grotesque horrors.
The atmosphere was bleak, dingy, and depressing.
Traveling through ruined cityscapes and then into cramped dark tunnels and then into alternate realities and the lower planes of hell, quite beautiful over all.
