Mothers' anguish over baby mix-up

Two South African mothers await DNA results after their babies were mixed up and one died, local media reports.

I still will never understand how hospitals can make a mix up like this. I was thinking most places had checks and measures in place to prevent this. I guess I was wrong because it dows not seem to include South Africa.
