3 killed as scaffolding collapses from high-rise in Raleigh, N.C.
Workers fell from an 11-story building under construction in city's downtown, the Associated Press, WRAL-TV and other media reported. A fourth person was sent to a hospital. Ref. USAToday

What's going on with all these building collapses all of a sudden? Once proper safety codes are followed along with regular safety site visits things like this should never happen.
