New SMF Custom Theme Form

We are getting so many requests for SMF Themes now that it was becoming monotonous to send out many Emails backwards and forwards to get the required information to begin. Most people do not like to take the time to properly read our Terms or FAQ so we needed a form that helps give the info to them a bit at a time. Therefore, we created a SMF Custom Theme Contact Form to make the process easier and understandable for all involved.
