So you're saying that he is not a mafia boss but he has dealings with mafia bosses? But then as the head of the country doesn't that make him a 'boss' over the mafia boss?

Not so much. Is a known issue in Italian political history that nobody can hope to become head of the Government without some sort of negotiation with the mafia. In the countrysides of Sicily and other southern regions, mafia has the power to channel thousands of votes to one candidate or the other. I let you imagine the rest. But that doesn't make a mafia boss of him. He, and anybody else, buys those votes. The payment usually consists in putting mafia controlled companies in charge of important public projects (building highways or bridges etc).
