Are Electronic Money Counting Machines really accurate or can they miscount?

I have often wondered this too. I have count all my change every time I have taken it to the bank. I do not tell the teller how much I have I just hand her the vessel all the coins are in. SO far it has been right on the money. I have done this over a dozen time now. I doubt I will count it next time.

They can obviously make mistakes but I think as long as the money is intact and genuine money they should be spot on every time.
I have never had enough money to need a counter to see how much notes I have though, I suppose they could be inaccurate if the notes are worn.
