Space Travel Safe? (Sorry Shuttle Victims)

Is space travel really safe? It seems that these kinds of accidents are more prevelant now with new technology than they were with the old. Its kind of like the Microsoft operating system... in the days of DOS no crashes, but now they put a layer of code over DOS, call it Windows and then it crashes your system, causes security loopholes, etc. Anyway, what are your thoughts?

REMAINS, PATCH FOUND

Human remains have been discovered in Texas among debris that rained down from
the skies Saturday after the space shuttle Columbia disintegrated nearly 40
miles above the Earth. Along with the remains, an intact mission patch, with
the names of the seven Columbia crew members, was discovered.

Well, in Bush's speech he says that this tragedy is not going to stop them to continue the space program. They don't know what it went wrong and I heard it will take long months to discover really what happened. It is so sad all this, they had only 16 minutes left before to land!

Someone was not doing their job or was no one listening?

From Deseret News:

ENGINEER WARNED OF SHUTTLE DANGER
Two days before the Columbia blew apart, a NASA safety engineer warned of the
possibility of "catastrophic" consequences if damage from the foam insulation
that struck the shuttle during liftoff allowed the heat of re-entry to penetrate
the wheel well and burst the shuttle's tires.

Maybe it was better for them that they did not know?

Engineers: We know that the shuttle could probably go through some serious difficulties, but who cares, we get good pay and human life is not that important.

I would do the same had I been managing any part of that flight.
