Patients With Bombs
I was watching a CNN report where a doctor working in Iraq told of detainees who had grenades and bombs implanted within them for future suicide missions. It was his job to remove these explosives. Can you imagine having to do that and at the same time saving theirs and your own life?

The doctor removes the device so that the person can use it, or it's his job to remove the device for disposal?
Can you imagine having a bomb or grenade implanted in your body? I would fear for it to detonate at any time!
If the doctor is removing the device for disposal, is the patient fighting against it? If the patient was willing to sacrifice himself in a suicide mission, why would he submit to having the device removed? I would suppose the doctor would have to sedate them by force. What a crazy situation.
